Corewell Health earns international recognition for its use of health information and technology to improve patient outcomes; first in Michigan to receive the honor since 2018
The health system is recognized as a 2024 HIMSS Davies Award recipient

Grand Rapids, Southfield, Mich., October 31, 2024 – Corewell HealthTM earned a 2024 HIMSS Davies Award of Excellence – the first health system in Michigan to receive the honor since Sparrow Health System in 2018. 

The Davies Awards program promotes HIMSS (Healthcare Information and Management Systems Society) to improve health and wellness through the power of information and technology.

Corewell Health received the award after submitting three evidence-based case studies demonstrating applications of clinical informatics to improve the care and wellness for its underserved and chronic disease patient populations across the state. 

The case studies submitted: 

  • High-Risk Underserved Clinic 

Corewell Health launched an innovative high-risk underserved clinic care model. The system levered tools within its electronic health record to empanel patients to the practice, identify high risk patients for care coordination and target patients who have a high risk of inpatient admission or emergency visit utilization. The high risk underserved clinic significantly reduced emergency department admissions, improved diabetes and hypertension control, and reduced total cost of care for this population. 

  • Longitudinal Care Management 

Corewell Health highlighted its longitudinal care management model, enabled by an innovative care management platform that guides care managers through addressing clinical and social determinants of health needs. Since the launch of the longitudinal care model, the 608 diabetic patients who graduated the program demonstrated an average 2.24-point A1c reduction and (for those empaneled patients who also had hypertension) an average 16.33-point improvement in systolic blood pressure. 

  • Reducing Readmission Risk 

Corewell Health leveraged predictive analytics and patient indicators for target populations to identify patients at the highest risk for readmission. Using these insights, Corewell Health deployed coordinated resources to focus on those individuals with unpredictable recovery journeys and who would most benefit from support of care managers. As result, Corewell Health reduced readmissions for the patients identified in the program by seventy percent.
